Under the direct supervision of the Pharmacy Information Services Manager, this position will provide clinical and pharmaceutical support to patients, hepatologists, and nursing staff through collaboration in an out-patient transplant clinic. This pharmacist is involved in managing drug approval, reimbursement, and clinical monitoring of clinic patients medication therapy. The pharmacist interacts with liver failure or transplant patients to evaluate referrals from providers. This pharmacist ensures qualification in the program and access to drug treatment. In addition, this pharmacist participates in ongoing monitoring through activities such as pre-authorization of prescriptions. This Transplant Clinic Specialty Pharmacist will establish protocols and tools for tracking and promoting patients medication use and compliance for achieving maximal clinical outcome. This individual will assess and document patients compliance or tolerance to drug regimen, along with clinical response to the treatment. The Pharmacist will provide clinical consultation and information regarding service and therapy management to patients, physicians or patient care team members as needed. The Pharmacist will maintain and manage the collection of quality outcomes data for the patient in collaboration with physicians and nurses. ()
